In accordance with Florida Statutes 112.9(j) and 112.91(i), Florida State University provides accidental death and dismemberment insurance to all Florida State University law enforcement officers. Coverage is provided for death or dismemberment due to an accident occurring while in the course and scope of employment. Suicide, attempted suicide or intentionally self-inflected injury is excluded.
Accidental death and dismemberment insurance premiums are assessed by the Florida Department of Management Services.
Rates are negotiated by the Department of Management Services depending upon the number of officers insured and the type of work performed. Premiums are paid by the officer's department.
As a State of Florida law enforcement officer, coverage is considered automatic. However, life benefits are paid to the officer’s designated beneficiary. Each officer must complete a Beneficiary Designation Form as soon as possible. You may obtain a copy of this form from your department representative or the form can be downloaded here. The completed form is maintained in your departmental personnel record.
